Extensive Experience with Complex Cases

At Prism, our extensive experience working with patients in residential treatment allows us to help people bridge the gap from these more structured environments to independence.  This also provides us insight into how to help other young people with mental health struggles to navigate this task and avoid more intensive and expensive treatment. 

Tailored Support at Critical Moments

When traditional once-a-week outpatient therapy sessions are not sufficient to stay out of inpatient facilities, we offer a more comprehensive and structured approach designed to provide individuals with the support they need while allowing them to continue functioning in their daily lives. 

Our approach is designed to empower individuals and families, promoting stability and well-being while minimizing the need for crisis interventions.

Our goal is to bridge the gap between standard outpatient care and inpatient hospitalization, reducing the cycle of repeated hospitalizations and worsening symptoms that too many patients experience.

Support Customized to Specific Needs

We believe that early intervention with additional resources can prevent crises and promote long-term stability. By integrating increased therapeutic support into a person’s routine, we help build resilience, enhance coping skills, and provide a safety net before more intensive care becomes necessary.

This foundational plan is tailored to meet each individual’s unique needs, ensuring that they receive the right balance of structure, guidance, and independence. While our standard program is listed below, we often customize the plan based on what is needed. No two patients are the same, and neither is their treatment!

Our standard program includes:

  • An initial T.A.R.G.E.T. Psychological Assessment to maximize program effectiveness
  • Multiple individual psychotherapy sessions per week to provide consistent, personalized support.
  • Several hours of community-based mentoring per week to help individuals apply coping strategies and life skills in real-world settings.
  • One psychotherapy group per week to foster connection, peer support, and shared learning.
  • One parent planning meeting per week to ensure family members are informed and equipped to support their loved one’s progress.
